Disclosure, Consent & Liability Agreement for a Session with Flowing with Marine, LLC

By purchasing any session or service offered by Flowing with Marine, LLC, I hereby represent and confirm in this agreement the conditions of my participation and agree to abide by the terms and conditions listed below. 

• I acknowledge that I am over 18 years of age and have voluntarily agreed to participate and request the owner of Flowing with Marine, LLC to facilitate an energy attunement and coaching on my behalf.

• I agree that any session or service is being undertaken with my full permission and that I am responsible for any mental, emotional, physical, or spiritual healing that may or may not occur as a result.

• I am aware that my participation in any session or service is not a substitute for psychiatric treatment, psychotherapy, therapeutic counseling, medication, medical treatment, or any other form of medical care or psychological therapy.

• I fully understand that the owner of Flowing with Marine, LLC is not a medical doctor and does not engage in the practice of medicine.  She has no formal training in medicine, psychology, or psychiatry and does not diagnose or offer a treatment or cure for any disease, physical or mental condition.  Always seek the advise of a qualified health care professional if you suspect you may have a medical condition. 

• During a session, a list of suggestions may be given, but I agree that before undertaking any suggestion, I will consult my physician or other health care practitioner whose care I am under.

• I acknowledge that by signing this form that I have not been hospitalized for psychiatric reasons within the last three years.

• I understand that a session with Flowing with Marine, LLC is not a psychic reading and is not meant to tell the future or give the understanding or impression that it can be used to create specific outcomes in healing or circumstance. 

I have carefully read and considered this agreement and fully understand its contents, terms, and significance.  I am aware that this agreement contains a release of liability between myself, Flowing with Marine, LLC and its owner.
